The Approach — What We Weighed

The pivotal question was never the dashboard — it was where the AI that reads a clinical encounter should run, and how much of it we should build versus rent.

Options on the table

• Off-the-shelf ambient-scribe SaaS — fastest to demo, but it routes visit audio through a vendor's cloud and hands over the note format, a hard sell under HIPAA.
• A fully self-trained model — maximum control over structure and data, but a research-scale cost and timeline no clinic would fund up front.
• A hybrid: managed foundation models under a signed business-associate agreement, wrapped in our own structuring, review and sign-off layer — control where it counts, without reinventing speech and language.

What we proposed

We chose the hybrid. The clinician-facing note schema, role-based access and audit trail are ours — the parts that carry compliance and the parts a clinic has to trust. The heavy language work runs on managed models covered by a BAA, so the practice gets accuracy without funding a model team. It kept time-to-value short, left the note format under the client's control, and made sure every AI-drafted line still ends at a human signature.
