The Challenge

Human services agencies are drowning in documentation. Direct support staff write hours of notes that nobody reads, while funders and regulators ask for outcome reports that nobody has time to assemble.

Existing systems were either compliance-first (and hostile to use) or easy to use (and not compliant). Agencies were stuck stitching the two together with PDFs and email.

Our Solution

OutcomeGrid lets staff capture hours, goals, behaviors and notes in the flow of work, then uses AI to roll those entries up into the outcome reports funders actually ask for.

We built it on HIPAA-ready infrastructure with 256-bit encryption and SOC 2 compliance from day one, so agencies don't have to choose between usability and audit posture.

The Approach — What We Weighed

The hard decision was never the capture form — it was where regulated client data would live and how AI could touch it without breaking HIPAA.

Options on the table

  • Adopt an existing compliance-first case-management suite — audit-ready, but rigid and hostile to the staff who fill it in every day.
  • Route notes through a general LLM API — fast to ship, but sending PHI to a vendor with no BAA was a non-starter.
  • Build a custom platform on HIPAA-ready, SOC 2 infrastructure with AI processing kept inside the compliance boundary.

What we proposed

We proposed the custom build. An off-the-shelf suite would have forced agencies back into the usable-or-compliant tradeoff that started the project, and a raw LLM integration put PHI — and their funding — at risk. Owning the stack meant 256-bit encryption end to end, role-based access, and an insights engine that rolls entries into funder-ready reports without client data ever leaving audited infrastructure — usability and audit posture in the same product.
